Polypharmacy represents a growing global healthcare challenge, particularly among elderly populations with multimorbidity. While multiple pharmacological therapies are often clinically justified, inappropriate prescribing increases the risk of adverse drug reactions, hospitalisation, and reduced adherence. Pharmacist-led interventions have emerged as a strategy to optimise pharmacotherapy and improve patient outcomes.
